If you do not wish to register On-Line, please follow these instructions:

1. Download the Registration Form page in MS Word 
2. Print a copy of the Registration Form page (make copies as needed)
3. Complete the Registration Form (If you plan to attend a practical session, please make sure you choose one that is not closed already).
4. Calculate your registration fee
5. Prepare a check payable to: PFDSSF   (Municipal Purchase Orders accepted. Registration must include PO Number)
6. Mail your registration form and check to:
